Discharge Assistance:  We will work directly with your physician, case manager, and/or hospital discharge planner to insure a smooth transition from hospital care to home care.

Patient/Client Instruction and Training:  A trained staff of rehabilitation and home medical equipment professionals ensures that each patient or client is fully trained on the safe operation and care of the equipment provided. Instruction is carried out in the convenience and comfort of the patient's or client's home.

Patient/Client Assessment:  A trained professional will meet with you to determine the needs of the patient or client with respect to the services and equipment provided/requested. Patients and clients are continually monitored and updated in regard to their equipment use to ensure that services are timely and up to date.

Delivery service:  Delivery service, setup, patient instruction and training is provided within the Twin Cities Metro area.

Consultations:  Staff professionals will meet, as necessary, with referral personnel or with the patient/client to resolve any matter involving home care equipment or service.
